About the Central West Self Management Program
The Central West Self Management Program supports people living with, or at risk for, chronic health conditions by providing the knowledge, skills and confidence to self-manage their chronic conditions and improve their health. The program is supported by the Ministry of Health and Long-Term Care and hosted by William Osler Health System. We serve the Central West Region (Brampton, Caledon, Dufferin County, Malton, Rexdale, and West Woodbridge).
What is Self Management?
Self-management refers to the activities that people can do to live well with chronic health conditions. These include gaining confidence to deal with medical needs, everyday roles & responsibilities, and emotional issues.
This is achieved by:
- Providing self-management education and skills training to people living with, or at risk for chronic conditions;
- Tips and tools to support caregivers; and
- Professional development for healthcare providers to enhance skills in self-management support
