Online Self-Management Program
The Online Self-Management Program is FREE to individuals across Ontario who are living with one or more chronic conditions such as diabetes, chronic pain, high blood pressure, stroke, cancer, heart or kidney conditions. Many of their workshops are also beneficial for caregivers and anyone looking to improve their health literacy.
Who should attend workshops?
Adults with any type of long-term health condition as well as their family, friends and caregivers are welcome to participate. The program is broadly accessible, allowing individuals with any level of computer skills to participate from any computer with an Internet connection, even a dial-up connection.

Physical Activity
- ParticipACTION: A Canadian non-profit organization that promotes healthy living and physical fitness.
- Virtual YMCA: The Y’s have created a virtual YMCA featuring home workouts, health and wellness tips, sto-ries, recipes, on-demand videos, activities for children and more!
Mental Health
- AbilitiCBT: An internet-based cognitive behavioral therapy (iCBT) program developed by Morneau Shepell that you can access from any device, any time. Now free for Ontario residents.
- BounceBack: A free cognitive behavioural therapy (CBT) program that offers guided mental health self-help supports for adults and youth 15 and older.
- Canadian Mental Health Association - Peel Dufferin Branch: 24/7 crisis support
- Crisis Services Canada: A national network of existing distress, crisis and suicide prevention line services.
- Mental Health Commission of Canada: A non-profit organization created by the Canadian government to study mental health, mental illness, and addiction.
- MindBeacon: Mental health support for people dealing with stress, anxiety, depression and more. Now free for Ontario residents.
- Wellness Together Canada: A collaborative project with various free mental health supports funded by the Government of Canada in response to the rise in mental distress due to the COVID-19 pandemic.
Nutrition
- Canada's Food Guide: Canadian guidelines to promote healthy eating and overall nutritional well-being, and support improvements to the Canadian food environment.
- Diabetes Canada - Recipes: A collection of nutritious, diabetes-friendly recipes provided by Diabetes Canada.
- Heart & Stroke Foundation - Recipes: Heart healthy recipes approved by Heart & Stroke dietitians that are high in fibre and lower in saturated fat, added sugars, cholesterol and salt.
Pain Management
- Pain U Online: An online hub of self-management modules developed by the Toronto Academic Pain Medicine Institute (TAPMI) to assist in managing chronic pain.
