Brief Action Planning
Brief Action Planning (BAP) is a highly structured, person-centred self-management support technique. This licensed workshop was developed by the Center for Collaboration, Motivation & Innovation. Composed of a series of 3 questions & 5 skills, BAP can be used to facilitate goal setting & action planning within the limited time available for appointments/interventions. Each skill of the BAP tool is based on literature & the evolving understanding of what works & does not work to help people change.
By the end of this workshop, participants will be able to:
- Identify clinician’s key role as partners and facilitators in self-management of chronic conditions.
- Implement specific and efficient communication strategies that can be utilized to support client-centered care.
- Demonstrate how BAP can help patients experiencing ambivalence and/or low confidence to set achievable action plans and problem solve barriers to change.
Who should attend?
Open to all health care providers, community support workers and healthcare students who support person-centered care.
How long is the workshop?
- 4 hours – Half Day (1 x 4-hour session or 2 x 2-hour sessions)
- Can be introduced as a 1-hour overview
