Brief Action Planning

Brief Action Planning

Brief Action Planning (BAP) is a highly structured, person-centred self-management support technique. This licensed workshop was developed by the Center for Collaboration, Motivation & Innovation. Composed of a series of 3 questions & 5 skills, BAP can be used to facilitate goal setting & action planning within the limited time available for appointments/interventions. Each skill of the BAP tool is based on literature & the evolving understanding of what works & does not work to help people change.

By the end of this workshop, participants will be able to:

  • Identify clinician’s key role as partners and facilitators in self-management of chronic conditions.
  • Implement specific and efficient communication strategies that can be utilized to support client-centered care.
  • Demonstrate how BAP can help patients experiencing ambivalence and/or low confidence to set achievable action plans and problem solve barriers to change.
