Chronic Disease Self-Management Program
The Chronic Disease Self-Management Program is
a 6-week workshop developed by Stanford University that helps people with
chronic health conditions manage their symptoms and daily lives. The program provides
people and their families with information and practical skills and tools to
help manage their health. It provides information designed to increase
confidence and motivation to manage the challenges when living with chronic
health conditions. This is an educational workshop and does not replace
the advice of your healthcare provider. No referral is needed.
What happens in a workshop?
Participants are encouraged to take an active role
in workshops by setting weekly goals and sharing progress in meeting goals.
Who should attend the workshop?
Adults living with chronic health conditions, as well as their family, friends and/or caregivers. The program is available in English, French, Hindi, Punjabi, Tamil and Urdu.
What does it cost to attend?
FREE to all! Participants receive the Living a Healthy Life with Chronic Conditions book.

Who leads the workshop?
The workshop is led by two trained peer leaders, who are often living with chronic conditions themselves or caring for someone with a chronic health condition.
