Diabetes Self-Management Program
The Diabetes Self-Management Program is a 6-week workshop developed by Stanford University that helps support the knowledge and skills people learn from their health care providers. This program helps participants build confidence and motivation to manage the daily tasks of living with type 2 diabetes. This is an educational workshop and does not replace the advice of your healthcare provider. No referral is needed.
What happens in a workshop?
Participants are encouraged to take an active role by setting weekly goals and sharing their progress in meeting the goals.
Who should attend the workshop?
Adults living with Type 2 Diabetes as well as their family, friends and/or caregivers. The program is available in English.
What does it cost to attend?
FREE to all! Participants receive the Living a Healthy Life with Chronic Conditions book.
How long is the workshop?
Participants meet once a week for 2.5 hours over 6 weeks.
Who leads the workshop?
The workshop is led by two trained peer leaders, whom have diabetes themselves or are caring for someone that does.
